WHO Health Status Survey & Health System Responsiveness in Egypt

Objectives
- Health status in different domains (Adult morbidity, adult mortality, birth history, various risk factors, and main chronic health conditions were examined through specific modules)
- The way populations report their health and value different health states
- Health expenditures and the modes and extent of payment for health encounters/intervention
- Assessment of the responsiveness of health system (what actually happens when the health system comes in contact with an individual (respect, dignity, confidentiality, communication, autonomy; prompt attention, amenities of adequate quality, access to social support networks, and the choice of institution and care provider)
- Through a nationally representative general population-based survey, the sample was selected from seven governorates (based on CAPMAS sampling frame)
- First mode of study was covered through a Household survey (6000 households). The International Classification of diseases (ICD) was used as the framework to gather information on mortality in the households and self reported morbidity. The revised International Classification of Functioning, Disability, and Health (ICF) was used to describe the essential elements of non-fatal health outcomes. The survey also included vignettes and some measured tests on selected domains that were intended to calibrate the way respondents categorized their own health.
- Second mode covered through a Postal Self-Administered survey (3000 respondents). Hand couriered approach was so efficient to the extent that the response rate was 92%
- Third mode used was the Key Informants survey (250 respondents)
- Fourth mode used was a Health Facility Survey (50 facilities: 25 inpatient & 25 outpatient health facilities) in 4 governorates
- Fifth mode used was Multi Method Valuation Assessment (500 respondents)
- Valuations involved the construction of summary measures of population health taking into account both fatal and non-fatal health outcomes.
- Measuring the value that individuals assign to descriptions of health states and to test if these varied across settings
- The health states were described as decrements in major domains of body functions and activities.
